You should submit a written report 2,500 words on a contemporary topic in patient
safety, apply learning from the module and draw on relevant literature and theory (70% of module
mark).
You should choose one essay question from the list below. Alternatively, if you have your own idea
for an essay relating to the Patient Safety module content, you can design a question and discuss
this with the module leader to confirm if it is acceptable.

What can and cannot be learnt from high reliability organisations in addressing and
improving patient safety in healthcare?
What are the challenges of the statutory Duty of Candour in promoting a culture of
openness within the NHS (or another healthcare system instead of the NHS, if relevant to
that system)?
Is there still a place for traditional root cause analysis within the NHS (or another healthcare
system instead of the NHS, if relevant to that system) and how does it influence patient
safety culture?
Who is responsible and accountable for patient safety?
