The purpose of this assignment is to
participate in a community health project. The project will follow the steps of
the nursing process from assessment, to problem identification, to goal
formation, to intervention, and finally evaluation. This is the major project
for this course.

1. Complete a Windshield Survey in your
community. Using the data gathered during the Community
Windshield Survey address the following questions in narrative format.

2. Using the PICO/T format, research and design
an evidence-based intervention to address one of the health disparities in the
community. For example, an educational intervention on oral hygiene
provided to preschool age children to address the high rate of dental caries. Use APA style formatting for in-text and reference list citations.U An educational intervention on hand hygiene provided to school age
children and adolescents to address the high rate of viral diseases (COVID)
