The patient is then discharged to her own home where a physical therapist from a home health agency comes in to train her to use a walker and build strength. After being at home for 25 days, Mrs. Smith develops deep venous thrombosis (blood clot in a vein) in her thigh (related to her fall) and is admitted to the hospital, from where she is transferred to a SNF after spending 2 nights in the hospital. Assuming that Mrs. Smith meets Medicare criteria for the services described here, how many days of SNF care is Mrs. Smith entitled to during her most recent stay (assume that she qualifies for the full 100 days)? How much in deductibles and copayments does Mrs. Smith have to pay?
She has Medicare A, B, D & Medigap: Aetna Part B. How much is her total monthly insurance costs for all 4 Medicare policies?(Medigap is $129.95/mo. and her Part D Aetna plan is $7.30/mo).

2nd Discussion: (Case Study) Mabel Brown resides in her own home and is admitted to a hospital following a stroke. After a hospital stay of eight days, Ms. Brown returned to her home, but she needs assistance with personal care and some speech therapy. Who will pay for the personal care services and speech therapy? She has Medicare A, B & D and Medicaid (Dual Eligible) Explain. If Mabel requires home help long term, will Mabel be eligible for services funded by the Older Americans Act? What services might she need? Will she pay for these?
3rd Discussion: (Case Study) Albert Johnson resides in an apartment and is admitted to a hospital following a heart attack and underwent coronary bypass surgery After a hospital stay of five days, Albert returned to his apartment, but he needs some assistance with personal care, shopping, and meal preparation. Part of Albert’s recovery includes changing the way he eats. Albert usually eats out at restaurants or picks up fast food. He understands he must eat differently and knows it will be an adjustment to address this. Albert has AARP Medicare Advantage Plan 1 (HMO) with United Healthcare. He just changed to an Advantage Plan earlier this year and was happy his doctor participated in this plan. Albert will have cardiac rehab at home for about a week and then he will go to an out-patient rehab facility for a number of weeks. What will all of this cost Albert out of pocket? His hospital stay, surgery, medications, dietitian, rehab and home helper for a period of time. He is worried about this.
