Patient, Population or Problem
The problem that I have found to address in my research is the problem of inadequate congestive heart failure (CHF) education and self-maintenance for patients. The proposed solution to this problem is for an educational and self-maintenance course given at discharge from the hospital.
What are the characteristics of the patient or population?
The characteristics of the patients are, patients that have been diagnosed with CHF, are English speaking, and do not have any cognitive disorders.

What do you want to do with this patient (e.g. treat, diagnose, observe)?
I would like to test the effectiveness of an education and self-maintenance course given at the time of the patients discharge from the hospital. This would be considered a preventative measure since it could help prevent a readmission of a patient. I want to observe the rate of readmission to the hospital in both the intervention group and the non-intervention group.
What is the alternative to the intervention (e.g. placebo, different drug, surgery)?
The alternative to the intervention group is a group that only receives the standard discharge process and standard follow up in their doctor’s office.
Outcome
What are the relevant outcomes (e.g. morbidity, death, complications)?
The relevant outcomes would be an increase in adequate self-care and decrease or in the re-admission rate to the hospital based upon the adequacy of self-care.
Ensure that the research question is answerable, feasible and clinically relevant
The research question is feasible and clinically relevant. CHF is a very prevalent issue and patients are discharged from the hospital with little to no knowledge of how to adequately take care of themselves at home.
My research would aim to find out whether an education and self-maintenance course would decrease re-admission rates to the hospital over a 6-month period. I have decided to focus on CHF education as a whole and explore the topic of improving self-care management of CHF. One such nursing research article I came across was aimed at designing and testing a mobile health application for patients with HF to monitor and track self-care management to potentially reduce HF-related readmission rates (Athilingam, Labrador, Remo, Mack, San Juan & Elliott, 2016). Another great article compared patients who were discharged home with a set of care-coordination strategies that included teaching patient’s self-care, medications and how to communicate with healthcare providers, while the control group was sent home with standard discharge instructions (Shaw, O’Neal, Siddharthan, Neugaard, 2014). These two articles were definitely helpful and both proved how necessary it is for patients to be able to take care of themselves after being discharged from the hospital in order to prevent readmissions.
