You are the school nurse at Smiling Learning Elementary School in your county of SWFL. Teachers are referring students to you to assess their nutrition and personal hygiene. In working with these children, you discover that about 15% are homeless and living in cars or shelters with their parent(s). Formulate a plan of care for a family in this situation living in their car. There are both parents, and 2 other children ages 3 and 15. What data would you want to gather to help you? Develop the plan to rehouse the family using local resources in your county. What potential sequelae might be anticipated in the children as a result of being homeless. What are indicators of success of your interventions? Describe your intervention at tier 1 level Quad Council competencies for this group. What level of prevention is it as supported by evidence/ research? What is the measurable goal for this intervention and how will you measure it?

Tier 1 Core Competencies: apply to generalist
community/public health nurses (C/PHN) who
carry out day‐to‐day functions in community
organizations or state and local public health
organizations, including clinical, home visiting and
population‐based services, and who are not in
management positions. Responsibilities of the
C/PHN may include working directly with at-risk populations, carrying out health promotion
programs at all levels of prevention, basic data
collection and analysis, field work, program
planning, outreach activities, programmatic
support, and other organizational tasks. Although
the CoL competencies and the C/PHN
competencies are primarily focused at the
population level, C/PHNs must often apply these
skills and competencies in the provision of
services to individuals, families, or groups.
Therefore, Tier 1 competencies reflect this
practice.
