Include a title page two blank evaluation tools that you have created, a summary describing your evaluation tool results with audience feedback, your updated grid (from module 3) and a reference page. Remember that this is to be submitted as one running WORD document. Please remember to use the 7th edition student paper format. To clarify on your evaluation tools – Create your own and submit two blank tools to show what you used for your audience feedback. Do not copy evaluation tools from websites. You may pattern yours from another source used as a guide, but your tool must be created by you.
Here are examples of what is required for each tool:
Content Tool: The content tool must measure content/outcomes – DO NOT USE A LIKERT SCALE for the content/outcomes tool. This tool is a post-test that measures what your audience has learned. Review your outcomes on your grid and create a tool that actually measures them. Examples: True/False, Fill in the Blank, Multiple Choice or Short Essay. NOT a Likert Scale or Self-Rating tool for Content. Create a short test on the topic you have presented using your presentation content.

List 3 predisposition factors that lead to falls:
Or
True or False: Only women are at risk for breast cancer.
Quality of Presentation tool: The other tool must measure quality of the presentation/speaker – this usually is done using a Likert scale.
Example question:
The speaker is knowledgeable on content: Circle 1 2 3 4 5 (5 being the best), etc.

Evaluation of Feedback summary: Include at least a paragraph (minimum 100 words) to discuss the methods used to evaluate your Project. Include why the method you have chosen to evaluate the project is appropriate. Include in your paragraph how you will take results from the evaluation to change or improve the presentation.
Final Grid and Full Reference page(s): Add your finalized grid from Week 3 with any corrections or updates needed, including the reference page.
